Duties

- Provide education and support to patients and their families regarding health management.
- Collaborate with interdisciplinary teams to coordinate care across various settings.
- Document all patient interactions accurately in compliance with healthcare regulations.
- Participate in quality improvement initiatives and adhere to safety protocols.
Skills

- Experience with ostomy care and neonatal ventilator management.

- Competence in home community care settings, including CT (Computed Tomography) procedures.
- Understanding of memory care techniques for patients with cognitive impairments.
- Strong lifting skills to assist patients safely when necessary.
