Position Summary :
We're looking for a culturally sensitive, enthusiastic full-time Peer Navigator for our Jackson Heights location. The Peer Navigator will assist with conducting outreach and engagement activities to recruit eligible clients; provide enrolled clients with additional social support e.g. navigation to healthcare systems. PN will also assist the Support Services team with tasks, such as appointment assistance, accompaniment, follow-up on care plan, emotional support, and assists with documentation of services in designated database system.
Job Responsibilities :
- Provide direct services to person who living with HIV / AIDS under supervision of the Program Manager for Support Services.
- Assist in carries out tasks that are needed to execute the support service plans, including the following :
- Accompanies patients to appointments when required,
- Provides coaching to patients,
- Assist with entitlements reassessment (i.e., health insurance),
- Coordinates logistics for plan adherence reminders, transportation, and / or childcare arrangement,
- Linking clients to primary care,
- Assisting in specialty referral,
- Field base outreach for lost to follow-up clients,
- Conducting home visit
- Track and documents client program participation.
- Maintain client-related records and other required documentation according to the protocols and standards of Apicha’s Department of Support Services.
- Working as a part of the unit at the quality assurance and continue quality improvement (CQI) tasks.
- Represent the agency at various venues to promote agency services.
- Other activities as assigned.
