We are currently seeking a Population Health Manager in our Manhattan location
Position Summary: The Population Health Manager will collaborate with the multidisciplinary staff to help meet the organization’s goals through the expansion of projects aligned with the mission of Apicha CHC and its strategic goals. The PHM will also support the implementation of strategies to enhance social determinants of health monitoring and drive the clinical transformation towards value-based care delivery. The role encompasses providing analysis, review, trends, and projections for key clinical metrics, supporting new program development, and making recommendations to ensure the viability of the organization over the short and long term. The position required to understand and maintain awareness of the changing healthcare landscape, including evidence-based practice, value-based payment, and delivery system transformation in order to achieve high quality of care.
Qualifications: Bachelor’s degree in nursing, Health Care Administration, or related field required; Advanced degree, in a related field such as MS, MPH, or MA strongly preferred; Familiarity with medical terminology, ICD-10, and CPT coding strongly preferred; Experience managing projects in health care quality improvement and management, performance improvement, and/or practice transformation preferred; Intermediate to advance analytic skills and knowledge of research methods, data collection, and analysis required; Experience with Electronic Health Records (EMR) and Population Health Systems preferred, Experience with eClinicalWorks strongly preferred; Strong working knowledge of Microsoft Office applications required; Ability to understand the principles of CMS, HEDIS, NCQA, and Health Plan Quality Standards required; Detail-oriented, excellent proofreading and editing skills required; Ability to work with multidisciplinary staff with diverse background required; Ability to adapt with constantly changing community health care field environment required
