What are the responsibilities and job description for the Social Worker position at aQua Dialysis?
- Assesses patient's social, psychological, cultural, and environmental status. Utilizes psychosocial evaluation to formulate a treatment plan as per regulatory requirement.
- Determines available resources. Communicates with Financial Coordinators and/or billing associates to assist patients with insurance applications. Determines patient need and eligibility for emergency funds. Maintains accurate records and receipts of use of funds as required by sponsoring agency.
- Assists with required patient admission forms within required admission and policy timeframe.
- Facilitates new patient orientation, providing dialysis options educational materials, and assists with decision making and the adjustment process.
- Provides assistance with education, advocacy, and referrals to community resources to all patients and their families, including transportation, medications, home care and financial needs.
- Supports patient and family in the adjustment to illness, treatment, lifestyle changes and promotes independence and wellness. Provides therapeutic emotional support to patients and works with patients on behavior change, including assisting the staff in dealing with violent/abusive patient behavior and facilitating behavior contracts if necessary.
- Documents patient data in the medical record. Assists in administration of patient survey process. Participates in C.Q.I, process as appropriate.
- Educates team members regarding the emotional aspects of chronic renal failure and related areas informally or through formal in-services.
