Overview
Responsible for coordinating the completion of the MDS , a comprehensive assessment instrument for each resident for the purpose of gathering information about the resident, developing a plan of care and monitoring changes in the resident's condition.
Responsibilities
Essential Functions
• Maintains current information regarding the resident's status and need for assessment; On admission; When there is a significant change in condition; For annual review; For quarterly review. • Assigns specific sections of the MDS to the appropriate department for completion, i.e., Social Services, Nursing, Physical Therapy, Occupational Therapy, Dietary, Recreation Therapy and Speech/Audiology Therapy. • Insures that Department heads and staff from all departments involved in the MDS Assessment are knowledgeable regarding their sections for completion by holding meeting's and classes to discuss various areas and answer any questions raised. • Schedules specific residents for review by assigning proper assessment data with with to use in carrying out assessments. • Insures that all sections are complete in a timely manner. • Signs in MDS booklet to certify completion of assessment. • Assists in the plan of care by helping to develop specific care plans for reach resident through a comprehensive care plan committee and through question and answer sessions with individual staff members of all departments involved in the MDS process. • Coordinates with the other disciplines to ensure that all information, services and care planning is done in a timely manner and to completion/transmission of the MDS Forms via email to the New York Department of Health. • Submits reports as required. • Communicates effectively on all levels both orally and in writing . • Maintains interdepartmental cooperation. • Analyzes, evaluates and solves inter and interdepartmental problems; recommends solutions when necessary. • Maintains and assures Quality Assurance Programs effectiveness. • Insures all code and regulatory requirements are met. • Follows organizational and departmental policies and procedures. • Maintains confidentiality of organizational and resident information . • Performs other related duties as assigned.Qualifications
Education & Experience
Formal EducationBachelor's DegreeExperience3 yearsLicense, Registration, and / or Certification RequirementYesMinimum KnowledgeRequires a broad knowledge of a highly technical or professional field. Requires highly complex conceptual thinking.Language AbilityAbility to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from groups of managers, clients, customers, and the general public.Education Requirements
• Graduate of an accredited School of Nursing.Education Desired
• Bachelor's Degree in Nursing preferred.Experience Requirements
• Two years experience. • Knowledge of MDS is necessary.Experience Desired
• Two years experience preferably in a geriatric setting.License Requirements
• Current license as a Registered Professional Nurse in New York State.Skill Requirements
