JobTitle: Care Manager Supervisor
Primary Function:
As a supervisory position, the position of Care Manager Supervisor assumes advanced care management skills. The CMS does not carry an active case load allowing for full attention to be given to supervisory duties with the primary responsibility of providing clinical skills and practical experience to a minimum of five (5) assigned care management staff. Under the supervision of the Program Director /Assistant Vice President, the CMS provides administrative support to the ACCESS Program.
Responsibilities include, but are not limited to;
· Uphold Argus Community policy and procedures (according to Argus Community’s Employee Manual). In addition, be familiar with ACCESS Program Policy and Procedures Manual
· Complete a minimum of 15-20 supervisory chart reviews per week
· Follow up as needed on charts that have been reviewed and discussed with the appropriate case manager within a two-week period
· Provide weekly/bi-weekly face to face supervision to assigned care managers and patient navigators
· Provide case specific supervision to care managers and patient navigators as needed
· Monitor the billing activity for assigned staff to ensure that minimum requirements are being met and that all billing meets Medicaid standards
· On a as needed basis participate in case conferences (both in-house and in the “field”) as a representative of Argus Community
· Assist with the training of new staff members (particularly those staff members assigned to teams supervised by the CMS
· Monitor sick, vacation, and personal times for those staff members assigned to teams supervised by the CMS
· Address disciplinary issues as related to common day – to – day activity of staff members with more serious infractions being brought to the attention of the Program Director or Deputy Director
· In consultation with the Program Director , attend appropriate trainings to enhance professional skill development
· At the request of the Program Director, represent agency at community, professional, and/or coalition meetings
· Meet with Program Director on a weekly basis for face –to- face supervision
· Perform other tasks as assigned by the Program Director
