Position Summary
The Case Manager is responsible for the case coordination of individuals with Intellectual disabilities and oversight of Host Home providers contracted with Ariel Clinical Services.

Essential Duties and Responsibilities
- Establish rapport to have an effective working relationship with individuals/families/guardians.
- Provide case management for individuals. Work closely with, and act as the liaison between collaborating agencies/multi-disciplinary team.
- Develop, monitor and document mandated services and programming, and ensure client satisfaction. Ensure individual files are audit ready.
- Conduct home visits, monitor and support host homes. Provide technical support.
- Participate in rotation for “on call” to support our individuals.
- Support individuals at Interdisciplinary team (IDT Team) meetings, and advocate as appropriate.
- In a timely manner, effectively and appropriately communicate potential risks and/or threats to an individual or program to Ariel supervisors and collaborating agencies.
- Participate in ongoing program evaluation to ensure program effectiveness
Additional Duties and Responsibilities
- Must work closely with other Ariel Programs and offices.
- Performs other duties as assigned.

Required Knowledge, Skills and Abilities
- The ability to effectively communicate verbally and in written format in both individual and group settings.
- Ability to market all services to the groups, agencies and individuals that Ariel serves.
- Ability to be flexible, and able to change direction rapidly.
- Ability to work independently, prioritize workload, complete a wide variety of tasks in a timely and accurate fashion and direct others, as needed
- Ability to anticipate, conceptualize and communicate an individual’s unique needs with the interdisciplinary team.
- Basic computer knowledge including: Videoconferencing knowledge and etiquette, and Microsoft Office products.
- Exercise independent judgment within the scope and boundaries of the department, the Agency’s Standard Operating Procedures, and Federal, State and Local Rules and Regulations regarding department operations.
- Ability to organize and present case material in a straightforward and concise written manner such that communication among treatment team members is clear concerning the individual/family.
- Knowledge of community resources (social, public agency, and information) in order to assist individuals in accessing these resources.
- Ability to provide reliable, insured transportation for self and others through the community served.
- Ability to demonstrate consistent and reliable attendance as scheduled and requested.
Education and Experience
Required:
- Bachelor’s degree in social or behavioral sciences, or a combination of four years experience and formal education in a Human Services field.
Preferred:
- Experience with evidence based individual behavioral supports and services.
- Minimum of two years working with I/DD population.
- One year experience mental health experience.
Certifications/Licenses
- Valid Regular Driver’s License, with acceptable driving record.
- CPR, First Aid – before contact with persons receiving services.
- Safety Care – within 60 days of hire.
- Informed Supervision –deviant sexual behaviors training - within 60 days of hire, may not be left unsupervised with PRS until this training is completed.
- QMAP – (Medical Administration Certification) within 60 days of hire, may not provide medication to PRS without this certification.
- Client Specific Training of individuals on caseload.
- BSIT

Physical Demands
- This position requires that the incumbent be capable of bending, sitting, stretching, and/or reaching regularly for periods up to 2 hours without a break.
- This position requires that the incumbent have adequate hand and finger dexterity to properly perform the functions of the position. Ability to type at least 35 words per minute.
- This position requires lifting regularly up to 10 pounds and occasionally up to 40 pounds.
- This position requires the ability to hear, see, smell and distinguish temperature, at a level which is adequate to ensure individual and staff health and safety. Adaptive devices are acceptable.
- This position may require daily driving of up to 1 hour per day and occasionally up to 8 hours per day.
Work Environment
- Exposure to typical climate controlled office environment when in the office.
- Exposure to all climates and weather conditions while working in the field.
- Exposure to blood borne pathogens and infectious disease is possible, but limited.
- Exposure to hazardous chemicals is possible, but limited.
