Position Summary:
The Licensed Practical Nurse (LPN) is responsible for providing excellent and compassionate patient care by assisting medical providers, educating and communicating with patients, ensuring continuity of care and promoting a healthy lifestyle.
Primary responsibilities:
- Assist providers with office exams and procedures.
- Provide patient education regarding health conditions during appointments and by phone.
- Triage telephone calls including lab and diagnosis results.
- Document all patient phone calls, lab, and diagnostic results in their electronic medical record (EMR). Consult with provider regarding any abnormal results for plan of care
- Facilitate specialist referrals.
- Perform routine nurse visits including administering vaccinations.
- Refill and place orders for medication as ordered or directed by the medical provider.
Required skills/education:
- At least a high school diploma or GED plus vocational training as an LPN
- Current, valid New York State Licensed Practical Nurse (LPN) license
- Current, valid CPR certification
- Valid NYS driver’s license
Nice to have:
- One to three years’ experience as an LPN preferred

- Per diem, must work at least 24 hours a month (3 shifts). No guarantee on amount of hours available.
- Monday – Friday day shift
- Hourly range $25-32

Physical Demands:
- Lifting Requirements 0-40 pounds
- Frequency of Lifting 0-25% of the time
- Average percent of time during regular shift devoted to: standing walking, bending, reaching and sitting 80%
- Ability to grasp with both hands; pinch with thumb and forefinger; turn with hand/arm and reach above shoulder height
- Ability to speak on the telephone and write simultaneously
- Ability to operate telephone system, simple lab equipment , computer system.
Visual, Hearing, Dexterity and Mental Demands:
- Vision: Adequate to perform the essential functions of the job such as reading medical equipment and computer terminal
- Hearing: Adequate to perform the essential function of the job such as answering the telephone
- Speaking: Adequate to perform the essential functions of the job such as communicating via telephone and in person for the purpose of relaying necessary medical information
- Dexterity: Adequate to perform the essential function of the job as it relates to performing simple medical procedures and testing
- Mental Demands: Adequate to perform the essential functions of the job such as pressure to maintain patient flow, dealing with patients and guests under stress or unhappy.
