At Home Solutions is looking for fee-for-service Home Care Field RN in the Buffalo area to provide high-quality patient care in the home. The RN must travel to the patient’s home to administer healthcare services and complete the required assessments.
Duties and Responsibilities:
- Conduct ongoing patient care and assessments.
- Initiates Plan-of-Care and makes necessary changes as necessary.
- Prepares written documentation and summaries of the patient’s status and progress as needed to continue home health services.
- Maintain close communication with the Director of Patient Services and the interdisciplinary team.
- Responds to phone calls and emails in a timely manner.
- Reporting all issues and concerns to the Director of Patient Services immediately.
- Completes documentation accurately and submits timely to the agency
- Provides the highest quality care in accordance with DOH regulations.
- Complete evaluation tasks, including reviewing medication and vital signs.
- Administer Physician-prescribed medication.
- Dress or redress wounds and assess the healing progress.
- Provide education to patients and families on proper home health care procedures and strategies.
Qualifications
- Graduate of an approved school of nursing
- Current NYS RN license
- Liability insurance is required.
- Excellent communication skills, detail-oriented, patient care, and assessment skills
- Able to function independently with minimum supervision.
- Demonstrates strong critical thinking skills.
- Ability to travel to patients’ homes.
- Long-term care 1 year preferred.
- Nursing home care 1 year preferred.
- Rehabilitation center 1 year preferred.
Schedule:
- Flexible, based on your availability (morning, evenings, and weekends)

- Rate: $100 per plan of care and $50 per HHA Supervision
Pay: $45.00 - $55.00 per hour
