Position Summary

We are currently recruiting a Behavioral Case Manager II. This full-time position pays $25.00 an hour and is located in Newark, DE. The schedule is 37.5 hours per week.

The State of Delaware, Department of Health and Social Services (DHSS), Division of Substance Abuse and Mental Health (DSAMH), is designated as the single state agency responsible for administering public behavioral health services in the State of Delaware. In accordance with Delaware Code, DSAMH is responsible for providing and overseeing public mental health and substance use treatment services for individuals 18 years of age and older.

This position is part of the Bureau of Community Behavioral Health and Social Determinants, under the Transitional Case Management (TCM) Mental Health Program. The TCM program provides individualized case management, temporary housing supports, and coordination of behavioral health treatment and recovery-oriented services for adult Delaware residents with serious and persistent mental illness, including individuals with co-occurring substance use disorders.

The Behavioral Case Manager II supports individuals through assessment, service planning, coordination of behavioral health and recovery services, advocacy, and collaboration with multidisciplinary teams and community partners to promote stability, independence, and overall well-being.

Essential Duties And Responsibilities

• Conducts client interviews and gathers case history from clients, family members, and other relevant sources.
• Provides guidance and information to families to support positive interactions and facilitate recovery.
• Evaluates client information and refers to appropriate services or resources.
• Prepares narrative, statistical, and court reports as needed.
• Participates as a member of a multidisciplinary team in assessing client needs and developing treatment plans.
• Prepares service applications, contacts referral agencies, and coordinates social service resources.
• Appears in court proceedings as required, providing testimony related to client circumstances.
• Provides technical guidance and consultative support to agencies and staff on client care findings.
• Advocates for clients in protecting rights and obtaining treatment consent.
• Uses computer systems and databases to document, track, and evaluate case data.
• Communicates effectively and respectfully with individuals of diverse backgrounds and abilities.
• Additional duties as assigned.

• Excellent organizational skills and attention to detail.
• Strong written, verbal, and interpersonal communication skills.
• Ability to work independently and manage multiple priorities.
• Demonstrated skill in case management, service coordination, and documentation.
• Proficiency in using automated information systems and data entry tools.
• Ability to work compassionately with individuals with serious mental illness and co-occurring conditions.
  
  

Education And Experience

• Bachelor’s degree or higher in Behavioral or Social Science or a related field, required.
• Minimum six (6) months of experience in case management, including assessing, planning, developing, implementing, monitoring, and evaluating services.
• Minimum six (6) months of experience providing clinical treatment, counseling, or determining eligibility for services as part of a treatment team.
• Minimum six (6) months of experience interpreting laws, regulations, policies, and procedures.
• Minimum six (6) months of experience in narrative report writing.
• Experience with State of Delaware Behavioral Health Systems, preferred.
• Valid Driver’s License, required.
  
  

Physical Requirements

• Prolonged periods of sitting at a desk and working on a computer.
• Standard hours of work Monday through Friday, 8:30 am – 5:00 pm, however, evening and weekend hours will be required.
• Travel may be required.
