Job Description
- Collaborate with the facility leadership and the medical director to develop, implement, and evaluate the annual infection prevention goals and action plan
- Identify occurrences, reservoirs, incubation periods, periods of communicability, modes of transmission, signs and symptoms, and susceptibility associated with the infectious disease process
- Ensure facility has a facility-wide system for the prevention, identification, investigation, and control of infections of residents, staff, and visitors, including surveillance designed to identify possible communicable diseases or infections before they spread.
- Assist the facility to conduct outbreak investigations and provide guidance to stop the spread
- Support the facilities in reporting possible incidents of communicable diseases to local, state, or national agencies as required.
- Foster effective working relationships and build consensus
- All other duties as assigned by supervisor
- A passion for caring for and improving the lives of our residents
- Current Registered Nurse (RN) License
- At least two (2) years of experience in a long term care environment.
- Must have or complete Infection Prevention CDC Certification.
- Good Working knowledge of State Rules/Regulations
- Positive Can-Do Attitude and Excellent Attendance
