RESPONSIBILITES:
- Develop, plan, implement and evaluate service program and service in accordance with state and federal guidelines.
- Identify and provide for resident's social, emotional and psychological needs
- Provide service coordination
QUALIFICATIONS:
- Licensed Social Worker, preferred
- One year of supervised social work experience in a healthcare setting, preferably with the psychosocial of the elderly
- Working knowledge of nursing homes regulations, including MDS procedures, and reimbursement and eligibility guidelines under Medicare and Medicaid
- Excellent communication skills; written and verbal
- Positive Can-Do Attitude
- Excellent Attendance
