Status: Full-Time / 72 h per pay period

The Social Work/Counselor-LISW/PCC provides direct social service intervention, transition planning and counseling to patients and their families. The Social Work/Counselor-LISW/PCC assesses the patient’s social, emotional, financial, cultural and family situation and interprets this information with the physician, nursing staff and other care team members to facilitate appropriate transition planning and clinical treatment. The Social Work/Counselor-LISW/PCC assists patients and families in making appropriate linkages to community resources and provides consultation to those seeking input and guidance in efforts to resolve patient concerns. Transition planning and psychotherapy services are provided to inpatient mental health patients in the forms of individual, group, family, couples therapy in accordance with evidence-based practice, the Ohio Administrative Code, and applicable standards of care. As prescribed in Ohio law, these services must take place during evenings and weekends 7 days/ week.
JOB QUALIFICATIONS
Education: Master’s Degree in Social Work or Counseling required
Licensure: Licensure with the State of Ohio Counselor, Social Work and Marriage and Family Therapist Board. LISW or PCC required, LISW-S or PCC-S preferred
Certification: CPI completed by end of probationary period. CPR completed by end of probationary period.
Experience: At Least 3 years of experience in a health care setting or other social service or behavioral health agency
Skills/Other: Must possess strong clinical and communication skills.
