AuthoraCare Collective is currently seeking an RN-Admissions. This position is Full-time: Monday - Friday and (1) weekend per month: 10:00am - 6:30pm supporting our Burlington service area.
Job Summary:
These functions/duties include but are not necessarily limited to the following: explains hospice services to patients and families and assist with the signing of consent documents. Coordinate, collaborate and develop plan of care with patient, significant others, and members of the interdisciplinary team to focus and assure patient centered outcomes.
Assesses, plans, implements, documents, organizes, prioritizes, teaches, delegates, supervises, and coordinates the care of adult and geriatric patients; (initial assessment, planning, coordination, evaluation, and teaching) which provides care and support to terminally ill patients and their families; and perform other nursing duties as requested.
