Description
- Use universal precautions/infection control measures and safety measures (proper body mechanics) during resident care,
- Prepare and/or review resident record documentation per facility policy and procedures (admission, care plan, transfers, charting for vitals, blood sugar, care giver shift report, etc.).
- Ensure procedures and treatments are carried out accurately per the care plan and documented appropriately,
- Notify responsible party, and health care professionals (doctor, therapists, staff, etc.) as deemed necessary about patient needs (rashes, skin tears, changes in behavior, eye problems, etc.)
- Refill medications as needed (insulin, eye drops, creams, pm, etc.)
- Perform self-medication evaluations with residents who have doctor orders to self-medicate.
- Complete and/or Review Incident Report form for completeness. Make appropriate
- contacts and document information has been adequately communicated to responsible parties and health care professionals (doctor, therapist, staff, etc.) Communicate Incident report findings to Director of Nursing.
- Use equipment and supplies effectively, safely, and efficiently per the care plan.
- Maintain personal hygiene and professional appearance as per facility policy dress code.
- Maintain resident and facility confidentiality at all times.
- Participate in on-going in-service education and staff meetings as directed by Director of Nursing
- Attend care plan meeting as directed by Director of Nursing
- Communicate with the Executive Director/Director of Nursing on a regular basis regarding employee availability (non-emergency PTO must be scheduled two weeks in advance to ensure appropriate staffing level are maintained for the residents)
- Represent Autumn Care Management, in a positive way, promoting good public relations with families, friends, and the community
- Attend approved training sessions, professional learning activities, and required meetings.
- Perform other duties as assigned as determined by the needs of Director of Nursing
Qualifications
1. License Practical Nurse or Registered Nurse
2. Demonstrated skills in the areas of Nursing Care
3. Understand Autumn Care Policy and Procedures and Employee Manual.
4. Ability to work as a team and supervise Resident Assistants effectively.
5. Computer literate.
