DUTIES AND RESPONSIBILITIES:
Housing Advocacy

  • Conduct comprehensive assessment; perform intake and enrollment as appropriate into the Transitional
    Housing Program.
  • Develop Housing Stability Plan with survivors that includes connections to applicable community and housing
    resources
  •  Assess survivor’s strengths, needs, abilities and preferences to empower them to develop permanent housing
    and service goals
  •  Assist in identifying and locating housing options suitable for survivors and assisting in the move-in process.
    Direct Services and Case Management
  •  Provide Case Management to survivors in the Transitional Housing Program to include goal development and
    assistance with accessing services that will lead to increase self-sufficiency and capacity to obtain and sustain
    permanent housing.
  •  Act as a liaison with other service providers.
  •  Provide all needed services to victims of domestic violence, but not limited to legal advocacy, medical
    advocacy, systems advocacy, Crisis Intervention including Safety Planning, Emotional Advocacy, Financial
    Advocacy, Employment Advocacy, Material Assistance, and Transportation.
  •  Maintain proper documentation of all contacts with persons in or seeking services. Follow record keeping
    procedures including documentation of abuse experienced and assistance provided, including maintain
    documentation of rent and utilities paid.
  •  Work cooperatively with all Avalon Center staff in making sure that the needs of persons seeking services are
    met.
    Collaboration with Systems
  •  Serve as a liaison with local officials and agencies for the purpose of reaching and assisting victims of
    domestic violence.
  •  Network and build relationships with community members, officials and agencies that may be able to
    positively impact the lives of victims.
  •  Identify and build relationships with potential landlords and utility companies.
  •  Connect with landlords and familiarize them with the Transitional Housing Program.
  •  Responsible for knowledge of and execution of the Transitional Housing Program including mainstream
    benefits, community wraparound services, and referrals
  • Other duties as needed may be assigned.

  • QUALIFICATIONS :
  • Minimum of a high school diploma or GED.
  •  Bachelor’s degree in applicable field or significant relevant professional experience preferred
  • Be available for on-call upon rotation
  •  Ability to respond quickly and effectively to many types of situations, including crisis-driven or potentially
    difficult situations involving clientele living with mental health conditions, engaged in drug/alcohol use,
    victims of crime, etc.
  •  Good communication skills.
  •  Ability to be a strong advocate and work within systems.
  •  Collaboration oriented.
  •  Strong commitment to ending power based violence and understanding of Trauma Informed Care.
  •  Ability to organize and coordinate people and activities.
  •  Reliable vehicle, valid driver’s license and current insurance.
  •  Maintain Confidentiality of Shelter, Shelter Residents And All Persons In Contact with
    Avalon Center Regarding Services.
  •  Physical Abilities:
    •  This position typically spends time standing, climbing stairs, driving, carrying and lifting (up to 25lbs).
    •  Frequent operation of computer keyboard, firm/strong grasping, and repetitive hand control. Able to
      complete all forms in personal writing.
    •  Make and receive telephone calls. Use of general office equipment, copier, scanner, and fax machine.
    •  Psychological tolerance for confrontation and/or disruptive behavior
    • LINES OF RESPONSIBILITY:
      Transitional Housing Case Manager is supervised by the Director of Community Outreach and Training who is
      supervised by the Executive Director.
      CLASSIFICATION:
      Full Time, Hourly, Non-Exempt
