Job Expectations:
- Performs the initial and ongoing Oasis assessments of the patient, determines the appropriate schedule of services for the patient.
- Communicates with the physician as needed to update patient status, clarify and/or receive verbal orders for patient care that then are entered into the EMR.
- Develops and implements an individualized plan of care for each patient, based on the assessment or evaluation, and in conjunction with the patient/ family, and physician.
- Observes signs, symptoms and changes in the patient’s physical or emotional condition and consults with the authorizing physician to modify careplan as needed.
- Records patient’s condition, interventions and evaluation in the electronic medical record as documentation of home visits, according to agency policy.
- Educate the patient/family/caregivers on disease process using teach back methods to ensure appropriate understanding to properly manage conditions upon discharge from services.
- Oversee all care of patient provided by LPN, completing supervision every 30 days..
- Develops aide care plan and supervises CNA every 14 days.
- Complete all documentation per agency protocols, Medicare/State of Michigan/Federal guidelines.
- Collaborates with the interdisciplinary team and physicians to ensure optimal care is provided while promoting continuity of care with appropriate admissions, transfers, and discharges
- Maintains prompt communication with interdisciplinary team and office in Home Health Notify system.
- Decrease re-hospitalization risk by prioritizing visits for high-risk patients, planning frequencies according to this need.
- Attend in-service trainings and mandatory agency meetings in accordance with agency policy.
Qualifications and Skills
- Ability to effectively work with a diverse employee/patient/physician groups
- Attention to detail is critical. Must be observant and follow directions from physicians and agency management.
- Must be able to perform physical and social assessment of patient and family.
- Demonstrate compassionate attitude and superior customer service skills.
- While Home Care experience is preferred we are willing to train those with no Home Care experience.
- This job offers a lot of flexibility, however you will work autonomously, so you need to be efficient in organizing and managing your time.

Requirements
- Experience with providing patient care within the home environment.
- Valid Driver's License
- Proof of Automobile Insurance
- CPR/BLS Certification
- State of Michigan Clinical License: Registered Nurse
- Current with TB Vaccination or Chest X-Ray
- Covid-19 vaccination or acceptable valid exemption
