What are the responsibilities and job description for the Community Health Worker - Non-Merit position at Baltimore County?

Regular Schedule: 34 hours per week

Personnel assigned to this classification may be required to work evenings and weekends.

A vacancy exist in the Department of Health and Human Services.

A list of eligible applicants will be established based on the examination as outlined below.

Current and future vacancies occurring in any Office or Department of Baltimore County General Government may be filled from the list of eligible applicants.

Under general supervision, performs routine community outreach duties for public health disease mitigation programs [Corona Virus-19 (COVID-19)]; provides education, and assists with case management. 
  • Conducts telephonic interviews of persons in need of COVID testing, vaccination, and engage in needed services. 
  • Provides basic education and information about disease-spread in a culturally competent manner.
  • Initiates home visits within two business days of assignment.
  • Works with Health and Human Services and other community providers to locate and link to care/services for hard to reach clients. 
  • Links clients to resources and distributes supplies to help facilitate adherence to shelter-in-place and disease containment.
  • Reinforces disease management protocols to reduce the spread of communicable diseases.
  • Documents client encounters and contacts made on the behalf of clients; maintains electronic spreadsheet of client encounters; completes and submits all reports within the required timeframes.
  • Performs miscellaneous job-related duties as assigned.
  • Works collaboratively and effectively within a team, provides feedback to other members of the team.  Builds and maintains cooperative and liaison relationships with public and private agencies.

Required Qualifications
Possession of a high school diploma or appropriate equivalent
plus
at least one year's experience which includes public interaction, outreach services, or working in a medical setting and/or medical terminology.  Ability to speak and write other languages preferred.  
 
Knowledge, Skills, and Abilities 
Knowledge of English grammar.  Knowledge of and sensitivity to cultural influences. 
 
Ability to learn and follow county and departmental rules, regulations, policies, and procedures; ability to establish effective working relationships with clients, public, and private agencies; ability to communicate effectively with individuals and groups; ability to work flexible hours; ability to work in a variety of environments; ability to maintain documentation regarding activities; ability to maintain records, files, and reports; ability to coordinate activities; ability to maintain inventory of supplies.

Licenses or Certificates
Possession of a valid driver's license equivalent to a noncommercial, class C Maryland driver's license is required at the time of appointment.

Applicants are required to submit proof of licenses, certifications and education beyond high school to meet the required and preferred qualifications of the position. Diplomas or transcripts must show the applicant's major field of study. Copies and unofficial transcripts are acceptable.

Proof of Degree Equivalency

EXAMINATION PROCEDURE
Applicants will be qualified based on an evaluation of their training and experience, as stated on their application, which includes answers to the supplemental questions.  Applicants must state the dates and duties of past and present experience clearly and completely for evaluation purposes.

Persons appointed to this classification may be required to work evenings and weekends.
 
The work of this classification requires traveling throughout the county, and therefore, requires daily availability of a vehicle. 
 
Training for this position will be provided; successful completion of the training is required.

Medical Examination and Employment Background Investigation
Applicants selected for an appointment to a position in Baltimore County must successfully complete a physical examination and drug screen and an employment background investigation, including, but not limited to a criminal background, education, and fingerprint check.
