Role Description
This is a full-time on-site role for a Case Manager at Banyan Heartland in Gilman, IL. The Case Manager will be responsible for coordinating and overseeing the delivery of individualized treatment plans, conducting assessments, collaborating with multidisciplinary teams, advocating for clients, and providing crisis intervention when needed. Sunday through Thursday, 8am - 4:30pm
