What are the responsibilities and job description for the Ambulatory Care Pharmacist position at Baptist Health Deaconess Madisonville?
Demonstrates consistent application of knowledge and skills to deliver appropriate, safe, efficient, and cost effective patient care as part of a multidisciplinary team. Collaborating on efforts to deliver patient care in a community-based care environment by expanding and enhancing pharmaceutical clinical-based services and assuring that the responsibilities outline below are carried out.
- Minimum Education, Training, and Experience Required
- Kentucky Board of Pharmacy license in good standing.
- Bachelor's Degree in Pharmacy or Doctorate of Pharmacy required.
- Minimum of 3 years related work experience in ambulatory practice preferred, completion of ASHP accredited pharmacy residency program preferred, or Board Certification in relevant practice area required preferred.
- BLS certification preferred
- Immunization certification preferred
