We are currently seeking an experienced Home Care Nurse for non-skilled home care. The Home Care Nurse will be responsible for assessing patient health, developing and implementing patient care plans and providing patient/caregiver education. The successful candidate will also ensure compliance with relevant laws and regulations.
Home Care Nurse Duties and Responsibilities
• Make In-person patient visits
• Provide patient-focused care, including accurately assessing needs, delivering or directing to the appropriate level of care, identifying potential health problems and influencing people to make better health decisions
• Document all patient/caregiver interactions via management software
• Provide annual caregiver training to improve performance
• Communicate with our agency's clients as needed and other team members - verbally
• Work independently to make clinical decisions on routine patient care matters
Home Care Nurse Requirements and Qualifications
- Bachelor’s Degree in Nursing or related field
- Current Registered Nurse (RN) license in the state of NC
- A minimum of Three(3) years as a Registered Nurse with recent direct patient care experience
- Excellent communication, interpersonal, and organizational skills
- Able to work independently and with minimal supervision
- Current CPR certification
- Knowledge of relevant laws and regulations
