JOB DESCRIPTION: Long-Term Care Experience a must!

  • Analyzing patients’ health status, goals and dietary restrictions
  • Creating individualized nutrition plans and meal plans
  • Educating and counseling patients and families on diet and nutrition
  • Tracking and assessing patients’ dietary intake and progress
  • Monitoring residents weight trends
  • Working with other healthcare professionals to coordinate care
  • Participation in inderdisciplinary meetings with clinical staff and department heads
  • Valid Registered Dietician for state of MA
