Job Summary:
The Oncology Nurse Navigator will act as a primary point of contact for cancer patients after initial diagnosis and until patient has sufficient knowledge and resources to self direct their care. The primary responsibility will be monitoring, coordinating care, and educating cancer patients and families as they move through the Bassett Healthcare system. The Oncology Nurse Navigator will contact cancer patients either in person or by telephone to establish a plan of care. It is the goal of the Oncology Nurse Navigator to assess the care and services required by cancer patients and make appropriate referrals. The Oncology Nurse Navigator will work in collaboration with social work and patient care coordinators for a smooth transition throughout the patients cancer care.
Minimum Requirements:
Education/Qualifications:Must be a graduate from as Associate or Baccalaureate degree program in Nursing.Experience:Minimum 3 years oncology nursing experience requiredPublic speaking experience is desired. Licensure/Certificates:Valid NYS RN license required.Current BLS certification is requiredCertification in Oncology Nursing must be obtained within 3 years from date of hire.Skills:Requires problem solving, decision making and critical thinkingRequires excellent leadership, organizational, written, and verbal communication and excellent interpersonal skillsMust be able to work in a self-directed environment, with ability to work with and lead teamsDemonstrates a high level understanding of oncology illnesses and treatment modalities. Skill in coordinating care needs across several disciplines.Demonstrates an ability to assess patient needs and make appropriate referrals as indicated for ancillary support services and programs.Demonstrates the ability to work in a team situation.Physical Requirements:Walking, standing, squatting and kneeling required throughout the shiftOccasional need for patient lifting and positioning.
