We are seeking a Palliative Care Coordinator to join our team and provide high-quality care to patients in their homes.
Responsibilities include:
- Coordinating patient care with interdisciplinary teams
- Developing and implementing individualized care plans
- Providing education and support to patients and families
- Monitoring patient progress and adjusting care plans as needed
Requirements include:
- Current RN licensure in NC
- Minimum one year of recent clinical experience
- Solid computer skills
