Palliative Care Coordinator

About the Position

This is an exciting opportunity to work for BAYADA Home Health Care, a leading provider of home health care services. As a Registered Nurse (RN), you will be responsible for providing palliative care to clients in their homes, while also collaborating with other healthcare professionals to ensure seamless patient care.

Key Responsibilities

• Develop and implement individualized care plans for patients
• Conduct thorough assessments of patients' physical and emotional needs
• Administer medications, perform wound care, and provide other medical interventions as needed
• Collaborate with other healthcare professionals to coordinate patient care
