Job Description: COMMUNITY SUPPORT SPECIALIST What you’ll get to do as a Community Support Specialist: Work independently to provide services in the clients’ homes and other community settings unless otherwise specified by the treatment plan. Encourage and promote recovery efforts, consumer independence, self care and responsibility. Participate in the ongoing development -of specific individual treatment/rehabilitation plans for each client on his/her caseload. Provide individual assistance to clients in accessing a variety of public services including financial and medical assistance and housing, including on an emergency basis. Use advanced knowledge of psychological concepts, evidenced based interventions and coordinating delivery of mental health services with other services provided by practitioners and community partners. Make ongoing assessments of the status of all clients on his/her caseload including personal strengths, progress, problems and needs. Provide individual assistance to clients in adhering to prescribed medication regimen. Education and Experience: Bachelor’s degree in Psychology, Social Work or other related human service specialized fields of study. OR... Bachelor’s degree in an unrelated field and two years of related work experience. OR... Combination of four years or more of advanced education and two years of related work experience. An Associate of Applied Science in Behavioral Health Support from an approved institution will also fulfill the educational requirements. Alternatively, four years’ work experience in a related field may substitute for educational requirements.
