Home Health Physical Therapist

">- Evaluate and assess patients' physical conditions and develop individualized treatment plans
">- Implement therapeutic exercises and techniques to improve mobility, strength, and coordination
">- Educate patients and caregivers on exercises, safety precautions, and self-care strategies
">- Collaborate with an interdisciplinary team to provide holistic, patient-centered care
">- Maintain accurate and timely documentation of patient progress and treatment plans
">Requirements:
">- Active Physical Therapist license in the state of Arizona
">- Doctorate or Master's degree in Physical Therapy
">- Minimum of one (1) year of experience as a Physical Therapist (home health experience preferred)
">- Current CPR certification
">- Reliable transportation with valid driver's license and auto insurance
