POSITION BRIEF
Assists individuals that are recovering from mental health and/or substance use disorders to gain the skills necessary for independent living. This position will become familiar with all Adult Residential programs / services to be able to assist team members as needed to ensure program and participants needs are met. Collaborates closely with all Adult Residential team members and is versatile to change locations quickly as requested.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Assist/provide support to program participants with various aspects of daily living skills and case management tasks
- Transport participants as needed to appointments, grocery shopping, etc.
- Assist in grocery shopping or Regional Food Bank deliveries for the community residences
- Work collaboratively and communicate effectively with team members to ensure resident needs are met including but not limited to coordination of mental health/substance use/primary care services, obtaining funding, transportation, linkage to other services, care coordination and discharge planning
- Assist in maintaining a safe and sanitary dwelling for residents
- Assist in developing, facilitating, participating in resident activities/outings
- Assist in facilitating and providing accurate documentation of monthly fire drills and safety checklists required for each location
- Promptly reports any facility/maintenance/safety issues to Program Supervisor to ensure a safe dwelling
- Provide restorative services and accurate documentation of services or statistics in a timely manner in accordance with Federal/State regulations and BHSN policies/procedures
- Participate in meetings and trainings as requested
- Additional duties as assigned within scope of job description
- Complies with all BHSN, state and federal regulatory policies and procedures
SKILLS AND QUALIFICATIONS:
- Excellent communication skills (both verbal and written) and interpersonal skills required
- High School Diploma or equivalent required;
- Experience working with those recovering from mental illness and/or substance use disorders preferred
- Complete NARCAN / CPR Training within 60 days of hire
- Ability to work a flexible schedule to meet the needs of program participants

- Valid NYS driver’s license for potential local travel
