Your Role

• Provide Critical On-Call Support: Deliver prompt clinical care and support in the hour hours setting to ensure the seamless continuity of hospice care.
• Respond to Patient Needs: Assess patient conditions and offer both clinical expertise and compassionate support during on-call shifts.
• Collaborate with the Interdisciplinary Team: Work closely with our team to coordinate personalized care plans tailored to each patient’s unique needs.
• Maintain Documentation: Ensure timely and accurate record-keeping in line with our high standards of accountability.
