Position Overview
We are seeking a dedicated and motivated Social Worker (Master’s or Bachelor’s level) to join our team. The ideal candidate will have at least 1 year of experience working with individuals experiencing substance use disorders or comparable clinical experience.
Responsibilities
- Support clients in accessing community resources and support services
- Collaborate with healthcare providers, mental health professionals, and community agencies
- Maintain accurate and timely documentation in compliance with organizational and state requirements
- Participate in team meetings, supervision, and ongoing training
- Assist in coordination with significant others to help in the treatment planning process whenever possible
- Facilitate of referrals to substance use disorder services as indicated by a client's medical provider, comprehensive assessment, or treatment plan
- Life skills advocacy and support accessing treatment follow-up, disease management, and education services, including referral and linkages to long-term services and supports as needed; and
- Receive continuing training in trauma-informed care and person-centered treatment planning
Qualifications
- Bachelor’s or Master’s degree in Social Work (BSW or MSW)
- Minimum of 1 year of experience in substance abuse treatment or related field
- Valid Minnesota Social Work License (or ability to obtain within 90 days)
- Strong knowledge of substance use disorders, treatment modalities, and recovery resources
- Excellent communication, problem-solving, and organizational skills
- Ability to work collaboratively in a multidisciplinary team
Preferred Skills
- Experience with motivational interviewing and evidence-based practices
- Familiarity with community resources and case management
