The Social Services Assistant's job responsibilities are to plan, organize, and direct the coordination of discharges with resident/patient, in accordance with current federal, state guidelines, as well as our facilities established policies and procedures, ensuring that the highest degree of quality care is maintained at all times.
1. The discharge planner is responsible for functions such as arranging referrals to other agencies (i.e. home care, medical equipment and outpatient services) including the provision of support documentation, as required by Medicare and/or insurance.
2. This person will work with the residents’/patients’ support network to facilitate a smooth discharge. They will assure facility goals meet the resident/patient and family goals; facilitating communication with appropriate team members.
3. Complete social history, discharge interviews and assess psycho-social needs as needed.
Requirements
Qualifications:
• Experience working with the geriatric population in a nursing home setting preferred but not required.
• Must be organized, have the ability to multitask and be able to set a pace necessary to meet discharge deadlines for patients.
• Must function independently and have flexibility, personal integrity and the ability to work effectively with residents, staff and
Support agencies.
• Must be able to communicate well with others either, in writing, by phone or face to face.
