GENERAL DESCRIPTION OF POSITION
Rehabilitation Coaches must be Certified as Qualified Behavioral Health Providers and deliver clinically appropriate interventions as ordered on the Individual Plan of Care. The Rehabilitation Coach works within a Treatment Team and under the direct supervision of a Mental Health Professional to provide therapeutic interventions with a Recovery-oriented, person-centered approach to assist members in re-building their lives.
QUALIFICATIONS
- Must have a High School diploma or GED and a valid driver’s license.
- Must be Certified as a QBHP and must have completed CSSP Training
- Must possess organizational, communication, and time management skills.
- Ability to understand member treatment plans and prescribed interventions.
- Self-motivated, able to work harmoniously in a team-oriented, culturally diverse environment.
- The ability to work in a constant state of alertness and in a safe manner.
- Must be able to do housekeeping duties, such as, but not limited to, laundry, sweeping, mopping, cooking meals, kitchen clean up, bathroom cleaning, etc.
ESSENTIAL DUTIES AND RESPONSIBILITIES
- Must work under the direction and supervision of a Mental Health Professional.
- Must complete QBHP certification and complete mandated continuing education training once per 12-month period to maintain QBHP certification.
- Effectively deliver and document interventions as prescribed.
- Documentation meets standards and shows that the LSC is providing services based on the members POC.
- Submit all documentation by the end of every shift.
- Late services cannot exceed 8%.
- Communicate daily with MHPs with any concerns/progress of members.
- Serve as a lead QBHP within the CSSP Program
- Serve as a liaison between members and community and other resources. Documentation needs to support connecting members with community resources with at least 12 documented non-billable collateral contacts, OP logs, or observations indicating liaison efforts within the evaluation period and 12 family contact observations for documenting communication with 911 monitors, family members, care coordinators, etc
- Assist members who wish to prepare for and secure jobs.
- Assist members in maintaining a safe, clean, healthy treatment/residential environment.
- Collaborate with other members of treatment team/other shifts assuring communication of vital member and other information.
- Serve as an advocate, resource, and role model for members.
- Maintain healthy relationships with members, always respecting appropriate boundaries.
- Delivered and documented interventions as prescribed for QBHP.
- Must complete Face to Face MHP Supervision every 14 days with a minimum of 12 contacts every 90 days and Monthly Observed Interventions.
- Adhere to Birch Tree requirements for attendance and punctuality.
- Possess the ability to transport members in company vehicles on an as needed basis and complete transportation notes for each transport.
- Use email communications to measure times issues were addressed.
- Must be able to work shifts assigned based on the needs of the members and/or branch. Every effort will be made to maintain a consistent schedule but there may times that schedules change on short notice.

- Regular and punctual attendance is required.
- A minimum of 12 Attended Supervisions. Expectation is 1 weekly supervision and 1 monthly observed intervention.
