Job Overview
We are seeking a compassionate and experienced Assisted Living Case Manager to oversee resident care and coordinate services within our senior living community. The ideal candidate will possess a strong background in long-term care, nursing, or social work, with a focus on assisting residents with diverse needs, including dementia and Alzheimer's care. This role requires leadership skills, operational management experience, and a dedication to enhancing the quality of life for our residents through personalized care plans and effective management. The Assisted Living Case Manager will play a vital role in ensuring compliance with care standards while fostering a supportive environment for residents and staff alike.
Duties
- Develop, implement, and monitor individualized care plans tailored to each resident’s needs, including those with dementia or memory impairments
- Coordinate medical and therapeutic services, including medication administration and health monitoring
- Supervise caregiving staff, providing leadership and guidance to ensure high-quality resident care
- Oversee daily operations within the community, including managing budgets and ensuring compliance with regulatory standards
- Collaborate with healthcare professionals, families, and community resources to support residents’ well-being
- Conduct assessments for new residents, ensuring smooth transitions into the community
- Manage documentation related to resident health, care activities, and incident reports
- Provide support and supervision for group home or retirement home settings, ensuring safety protocols are followed
- Lead efforts in staff training related to Alzheimer's care, dementia management, and other specialized needs
- Engage in community outreach to promote services and foster positive relationships with residents’ families
Experience
- Prior experience in assisted living facilities, nursing homes, or long-term care environments is essential
- Demonstrated working knowledge of Alzheimer’s care, memory care, and dementia management
- Proven supervisory or management experience within senior or group home settings
- Familiarity with home & community care operations and home care services is highly desirable
- Background in social work or nursing with a focus on senior or developmental disability populations is preferred
- Skills in budgeting, financial acumen, and operations management are advantageous
- Experience administering medications and understanding healthcare regulations related to long-term care is required
- Strong leadership abilities combined with excellent communication skills to effectively manage teams and engage with residents’ families
Job Type: Full-time
Pay: $23.34 - $28.11 per hour
