    • Working on-site at the BJC Learning Institute 
    • Support teams through authorization coordination
    • 5 to 10 years of med/surge, long term care or hospital experience highly preferred.

Preferred Qualifications

Role Purpose

The Post-Acute Care Coordination Specialist interfaces with hospital care coordinators and care teams, as well as external agencies that may include post-acute partners and payers, to provide support and facilitation of safe and expedient patient care transitions across the continuum of care. Responsibilities include gathering, organizing and transmitting information to communicate essential care transition information to key stakeholders to accelerate post-acute care transition processes.

 

Responsibilities

  • Responsible for processing care coordination requests in a timely manner in accordance with the established policies and criteria.
  • Completes documentation and consistently communicates with the appropriate team members and external agencies within the established timeframes to facilitate continuity of care.
  • Establishes effective working relationships with team members, third party payers, care coordinators and post-acute care representatives.
  • Maintains current knowledge of regulatory, reimbursement and third party contract guidelines.
  • BJC has determined this is a safety-sensitive position. The ability to work in a constant state of alertness and in a safe manner is an essential function of this job.
  • Minimum Requirements

    Education
