We are seeking a Registered Nurse to join our team of home health and outpatient professionals. The successful candidate will be responsible for providing patient care in a variety of settings, including home health visits, outpatient clinics, and hospital visits. The Registered Nurse will be expected to assess patients' needs, develop and implement nursing care plans, and provide patient education. The ideal candidate should have excellent communication skills and the ability to work independently.
Responsibilities:
- Provide direct patient care in a home health setting.
- Provide education and counseling to patients and families.
- Perform assessments and administer medications as prescribed.
- Develop and implement patient care plans.
- Monitor patient progress and document changes in condition.
- Collaborate with other healthcare professionals to ensure quality patient care.
- Provide support to patients and families in an outpatient setting.
