What are the responsibilities and job description for the Case Manager position at Boca Recovery Center?
- Complete a case management assessment;
- Meet with clients to assess their ancillary needs and strengths including medical, psychological, employment, financial, family and other social functioning, etc.
- Communicate by phone or written letters with employers, judges, lawyers, probation officers, landlords and family members.
- Knowledge of resources available in the community to help clients with things such as transportation, childcare, employment, etc.
- Assist clients with scheduling medical, dental and other ancillary appointments
- Take care of FMLA paperwork or unemployment papers as needed.
- Assist clients applying for Food Assistance when applicable
- Help clients to manage money and assist with paying their bills while in treatment.
- Monitor phone calls as required
- Assist clients in looking for employment and preparing resumes.
- Teaching life skills such as budgeting, critical thinking, employment, interviewing, communication, social, personal care, respect, etc.
- Develop and assist with discharge planning for aftercare such as: booking airline tickets, setting up OP and counseling appointments, and halfway houses post discharge
- Advocating for clients, detailed record keeping, motivate and encourage clients in non-judgmental manner
- Requires complete knowledge of the organizations related programs, practices, and policies as well as rules of documentation and confidentiality.
- Responsible for assisting clients with the discharge planning and linkage to continuing services.
- Prompt and regular attendance
- Other duties as assigned
Salary : $53,800 - $68,100
