Registered Nurse/LPN

Responsibilities include:

1. Plan, treat, organize and case manage assigned patients.

2. Provide exceptional patient care to successfully transition back home and prevent re-hospitalization.

3. Provide home health skilled care, disease management, education, and documentation and coordination with other discipline.

4. Attend company in-service and training to keep up with industry changes and requirements.

Requirements:

• Nursing state licensed in Illinois.
• Compassionate, dedicated and ethical.
• Self-motivated, self-directed

Job Types: Full-time, Part-time and PRN

Pay: $40.00 - $100.00 per visit

Experience:

• Nursing: 1 year (Preferred)

License/Certification:

• BLS Certification (Preferred)
• RN (Preferred)
