The Care Coordinator is responsible for ensuring a whole person health approach to address behavioral health and wellbeing of members served. The Care Coordinator arranges the care for members served and facilitates the sharing of information and care among all care team members including the member, family, behavioral health and physical health providers, and other community support systems to ensure appropriate, effective use of services ensuring members care at the right time with the right resources. Services delivered by the Care Coordinator are recovery-focused and include coordination of care, resource identification, transitional support, development of supports and resources, and system navigation.

Responsibilities

Essential Functions:

  • Manages Case Load for Intensive Care Coordination program for the Idaho Behavioral Health Plan
  • Works collaboratively with all care team members to reduce barriers and actively engage members
  • Conducts comprehensive screenings in collaboration with the Clinical Supervisor to develop individualized care plans that are inclusive of all behavioral health and whole person needs
  • Consults and collaborates with member, family, behavioral health providers, primary care provider and all identified on the member’s care team to ensure necessary care and supports are provided
  • Coordinates and facilitates care team meetings
  • Connects clients to community resources inclusive of all social determinants of health
  • Provide member education and treatment options to promote the members self-management
  • Provide transitional support services to ensure appropriate transitions from one level of care to another to help prevent duplication and recidivism
Qualifications

Experience and/or Education:

  • Bachelor's Degree in Health and Human Services or another similar related field required (Psychology, Sociology, Social Work, Child Development, Counseling, etc.)
  • Preferred experience in providing care coordination or case management
  • Preferred experience working with children’s mental health
  • Preferred knowledge of behavioral health issues and diagnosis
  • Knowledge of community resources and/or willingness to learn

  • Knowledge of utilization management procedures and behavioral health community resources and providers
  • Ability to exercise sound judgment, make logical decisions and to perform independently and as a team member
  • Detail oriented and accurate with the ability to manage multiple projects at one time
  • Strong initiative and creative problem-solving abilities
  • Appreciation of cultural diversity and strong sensitivity towards target member population

  • Must be reliable, punctual, and flexible
