What You’ll Do
As a Nurse Navigator at Brandon Wilde, you’ll play a vital role in guiding residents and their families through complex healthcare decisions and transitions. Your responsibilities will include:
Initial Assessments
- Conduct comprehensive evaluations of new residents’ health status, medications, and care needs.
- Identify potential risks such as fall hazards, cognitive decline, or chronic condition complications.
- Collaborate with physicians, therapists, and caregivers to create and update individualized care plans.
- Manage scheduling for medical appointments, lab work, and specialist consultations.
- Educate residents and families about diagnoses, medications, and treatment options.
- Serve as a strong advocate for residents’ preferences and well-being.
- Assist residents during transitions between care levels (e.g., independent living to memory care).
- Coordinate hospital discharges and ensure smooth post-acute care follow-ups.
- Help residents and families access community resources, financial aid, and support services.
- Provide guidance through insurance and Medicare/Medicaid processes.
- Offer emotional support during health challenges and end-of-life planning.
- Facilitate family meetings and care conferences with empathy and clarity.
- Track residents’ health status and intervene proactively to prevent complications.
- Promote continuity of care and reduce unnecessary hospitalizations.
- Associate’s degree in nursing from an accredited school of nursing with a current and valid Georgia RN license and BLS certification.
- Three to five years of experience working in long-term care, post-acute care or other health care setting.
- Excellent verbal and written communication skills for groups and individuals of different populations including persons with disabilities and dementia.
- Ability to work collaboratively with community management, internal and external resources/partners and communicate with residents, families, physicians, and staff.
- Comfortable using technology such as Microsoft Office and Electronical Medical Systems, ex. Point Click Care.
- Effective organizational skills including delegation, managing multiple priorities, time management.
- Team Collaboration: Work side-by-side with a full-time Nurse Navigator who values partnership and shared purpose.
