Care Coordinator

Job Summary: The Care Coordinator will provide therapeutic case management and work collaboratively with inpatient social workers, treatment teams, and Utilization Management to ensure timely coordination of inpatient care, treatment planning, and discharge planning. This position requires strong organization skills, the ability to work with the interdisciplinary team, strong documentation skills, and competency in the use of the electronic health record.

Qualifications

• Minimum of a Bachelor’s degree in human services or related field required.
• Minimum of two years of experience working in a human services field.
  
  
  

Core Standards & Competencies

• Will meet with patients at the direction of the attending social workers to complete tasks that support inpatient treatment and aftercare goals.
• Will ensure that treatment plans are completed in the required timeframe, will attend treatment team and scribe treatment plans and updates, will review treatment plan with patients/guardians and request signature..
• Will collaborate with outside providers, referral sources, and serve as primary contact for families and guardians.
• Will ensure comprehensive aftercare planning (including providing education to patients and guardians about aftercare resources, scheduling medical and mental health follow up appointments, and completing prior authorization for medications as needed) begins at admission and continues through the patient’s stay.
• Will follow up with a phone call to patient or guardian 2-7 days after discharge to ensure that aftercare needs have been met, offer additional guidance for accessing services as needed.
• Demonstrate a basic knowledge of community resources related to but not limited to mental health, substance use, economic assistance, housing options, and transportation.
• Provide clear and concise documentation in the form of a Contact Note outlining contact with outpatient team members, families, and guardians during inpatient treatment.
