- Provide community-based case management services to support clients in achieving goals and objectives identified in needs assessments and rehabilitation plan
- Advocate, counsel, and coordinate daily activities for clients in the area of independent community living
- Provide input to the needs assessments and rehabilitations plans
- Bachelor's Degree in a Human Services field
