Job Overview
We are seeking a compassionate and skilled Licensed Vocational Nurse (LVN) to join our healthcare team. 1:1 Care | Stable Schedule | Full Time (40 hours per week)
San Jacinto, CA - Bilingual in English and Spanish Preferred
Pay Range: $31-$33 hourly with paid lunch breaks

We are seeking a Licensed Vocational Nurse for shift care to join our team! You will be responsible for delivering high quality care to assigned patients.
Responsibilities:
- Provides care/services to patients including the administration of treatments and medications according to the patient’s plan of care.
- Prepares clinical and progress notes. Maintains accurate and timely documentation of care/services provided.
- Collaborate with the Registered Nurse on patient care and any change as well communicate with other team members, as appropriate, regarding the patient’s status.
- Assists the patient in learning appropriate self-care techniques. Provides appropriate patient education and teaching materials.
- Reviews the patient’s medications; updates the patient’s medication profile when there are new, changed, or discontinued medications; and notifies the Registered Nurse of medication changes.
- Attends and participates in staff meetings, educational in-services, and committee meetings, as appropriate.
- Reports adverse events, incidents, and grievances/complaints (patients and/or personnel) to the Clinical Manager.
- Participates in the agency’s Quality Assessment and Performance Improvement (QAPI) program, as appropriate.
- Complies with all applicable federal, state, and local laws and regulations in performance of job functions.
Qualifications:
- Previous experience in nursing or other medical fields.
- The employee must occasionally lift and/or move up to 30 pounds.
- Familiarity with medical tools and equipment.
- Ability to build rapport with patients.
- Compassionate and caring demeanor.
- Excellent written and verbal communication skills for documenting charts.
- Must have a CA Driver's License, Licensed Vocational Nurse, and Basic Life Support Certificate.
