What are the responsibilities and job description for the Community Support Program Case Manager position at Bristol Bay Area Health Corporation?
PURPOSE OF THE JOB: To assist the Community Support Program (CSP) Coordinator in providing outreach, case management, psycho-social development, vocational rehabilitation, and residential resources to consumers in Bristol Bay diagnosed with a severe mental illness.

- Bachelor’s degree in the area of Psychology, Social Work, Public Health, Public Administration or other human service field required
- Two years’ experience working in the mental health field, preferably with severely mentally ill clients preferred.
- This employee must be able to demonstrate the knowledge and skill necessary to provide care appropriate to the age of the patients served in his/her assigned unit. The individual must demonstrate knowledge of the principles of growth and development over the life span (e.g., pediatric/adult/geriatric) and possess the ability to assess data reflective of the patient’s status and interpret the appropriate information needed to identify each patient’s requirements relative to his/her age specific needs, and to provide the care needed as described in the assigned unit’s procedures.
