$25.00 based on experience

Our Social Worker will function as a lead member of the interdisciplinary team, assisting in the creation of discharge plans for our sub-acute rehab patients.

We are always looking for positive, dependable, caring individuals to join our team to meet the growing demand in our community for quality care. Social Worker duties include:
- Completion of psychosocial assessment
- Development of care plans
- Educating and completing advance directives with patients/families
- Coordinating discharge plans, connecting patients/families with community resources
- Educating patients/families on alternate levels of care
- Completion of MDS 3.0 assessments

Qualifications
- Minimum requirement of a Bachelor’s degree in Social Work (BSW) or related field from a college or university with an accredited Social Work program
- One (1) year experience in a SNF setting
- Must have experience and/or training in the field of geriatrics
- SNF Discharge planning experience preferred
