Job Description
Responsibilities
This position works in partnership with the clinician to serve the patient, and patient’s family through the coordination of discharge planning.
Discharge planning includes coordinating transportation, setting up aftercare appointments, and faxing or scanning the discharge information to the aftercare provider.
This person must possess discretion, the ability to act independently, good clinical judgment, creative thinking and problem-solving initiative.
Strong communication skills, and the ability to exhibit empathy are a must. The person in this position must be able to demonstrate strong organizational skills and strong time management skills.

Qualifications
MINIMAL QUALIFICATIONS
Education :
Bachelor’s degree in social work, counseling or a behavioral health related field.
Experience :
Two years’ experience in a social services setting in behavioral healthcare environment preferred.
Physical requirements :
Medium Work Exerting pounds of force occasionally or pounds of force frequently or an amount greater than negligible and up to pounds of force frequently or an amount greater than negligible and up to pounds constantly to move objects.
Physical demand requirements are in excess of these for light work. Requires walking or standing to a significant degree.
