WHAT YOU CAN EXPECT Collaborate with multidisciplinary teams to evaluate and assess patients’ medical and physical needs. Refer patients to appropriate services if patients are identified with depression, anxiety, grief or mental illness. Utilize consultation data and social work experience to plan and coordinate client or patient care and rehabilitation, following through to ensure service efficacy. Identify environmental impediments to patient progress through interviews and review of patient records. Assist patients and their caregivers in identifying goals of care and assists with unmet needs through linkages to community resources. Assist in crisis intervention. Document patients’ electronic medical records in a clear, concise, accurate, and timely manner.
