NOTE: This job will start as PT 3 shifts per week and could move to 4 quickly. We also plan to have more need for full-time LVN's as we expand later in 2026.

Overview

Position Summary

The LVN is part of the team concept in a clinical setting for the delivery of healthcare. This position oversees client medications via monitoring, assessing clients for mental health, builds rapport with clients, follow-up on medications and doctor appointments.

Essential Duties & Responsibilities

The essential functions include, but are not limited to the following:

  • Follow Physician's instructions on medication management protocol and procedures.
  • Monitor clients’ physical and emotional well-being; vital signs checked per physician order.
  • Monitor clients’ vital signs for signs/symptoms of medication reaction.
  • Write notes and reports, as protocol and procedures indicate.
  • Document and report on the client's status to team members.
  • Utilizes HIPAA compliant messaging platform to communicate activities in operations and collaborate with other departments. This includes reporting medication management, medical issues, crisis intervention, conflict resolution, compassionate support, and must include any attempts to resolve current issues.
  • Completes all items on the LVN Weekly Task List and confirms completion with supervisor.
  • Supports BHT’s in obtaining urinalysis (UA) for new admissions and weekly UA’s that are sent to the diagnostics laboratory.
  • Complete nursing assessment on clients admitted to the mental health residential program.
  • Complete transcription of doctor’s orders pertaining to modified protocols.
  • Assist clients with all self-administered medications via monitoring as directed by the appropriate health care professional (i.e. Medical Director).
  • Assist clients with personal hygiene and other ADL’s.
  • Attend job-related meetings, in-service training, staff development training, participate in team problem solving methods, building rapport, respond to requests, and maintain a cooperative relationship among the health care team by communicating information.
  • Attends weekly operations meeting to report on operations activities, help improve systems, receive training/supervision on pertinent topics.
  • Maintains nursing supplies inventory by checking stock to determine inventory level, anticipating needed supplies and informing supervisor so that they may place order(s) for supplies.

Minimum Qualifications

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/ or ability required. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.

All direct care staff shall have graduated from high school or possess a GED and have a minimum of one (1) year of full-time experience, or its part-time equivalent, working in a program serving persons with mental disabilities. Such experience must be in the direct provision of services to clients. If the employee does not have the required experience, the program shall document a specific plan of supervision and in-service training for the employee which will guarantee the ongoing qualification of the employee to perform the job. The plan should include but not be limited to the frequency and number of hours of training, the subjects to be covered, and a description of the supervision to be provided.

  • High School Diploma required; Bachelor's Degree preferred.
  • Current LVN license in State of operations required.
  • CPR and First Aid Certified.
  • Two years of experience as an LVN or in a health-related field.
  • Must be 18 years of age
  • Must have required health screening
  • Must have current TB
  • Must have criminal record clearance

  • Knowledge of medications effects, indications, and contraindications.
  • Ability to read and interpret written information; write clearly and informatively; edits work for spelling and grammar. Ability to speak clearly and persuasively in positive or negative situations; listens and gets clarification; responds well to questions; demonstrates group presentation skills; and participates in meetings.
  • Working knowledge of Microsoft Word, Excel, and Outlook.

Physical Demands and Work Environment

The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this position. Reasonable accommodations may be made to enable individuals with disabilities to perform the functions.

Walking, bending, and kneeling which may occur with long periods of sitting in vehicle while transporting; must be able to participate in moderate lifting of 30 or more pounds (suitcases, packages, etc.). This position must be able to support client weight in case of emergency situation or client disability requiring assistance in walking, etc. There is a risk of exposure to bodily fluids infections, contagious / blood borne diseases, and chemicals (cleaning agents), an exposed risk to highway accidents, and an exposed risk to varying, unpredictable situations (i.e. client behavior).

Qualifications

  • High School Diploma required; Bachelor's Degree preferred.
  • Must be 18 years of age.
  • Must have a required health screening.
  • Must have current Tuberculin Screen or obtain upon 7 days of hire.
  • Valid CPR/First Aid Certification.
  • Must have criminal record clearance.
  • Must be able to work overtime, weekends, holidays, and flexible hours.
  • Must have a valid driver’s license and clean driving record.
