Job Type: Per Diem position with mileage reimbursement.
Coverage area includes: Inland Empire, San Bernardino, Redlands, Riverside, Beaumont, Temecula
Job Description
The hospice LVN cares for terminally ill patients in their home under the supervision of an RN and adhering to an individualized plan of care. This includes implementing and educating both patients and families on end of life process, medication administration and safety, safety in the home, treatments per MD order, etc.
- Needs to be organized to manage patients
- Needs to communicate well with administrative and field staff
- Needs to document appropriately and follow company policy and procedure
- MUST have experience working with facilities.
Required work experience:
- 1 year prior experience as a hospice LVN
Additional Duties:
- Participate in PRN visits as needed.
Required Qualifications:
- Currently licensed as a LVN in the State.
- Three years nursing experience.
- Current BLS provider card
- Great verbal and written communication, and organization skills.
- Must be a licensed driver with an automobile that is insured in accordance with state requirements
- Basic computer competence

Pay: $28.00 - $33.00 per hour
