Registered Nurse (RN) – Home Health / Hospice (Travel Contract)
Contract Length: 8 Weeks (Potential to extend to 12 weeks based on census)
Location
Greensboro, NC
Service Area: Greensboro & High Point, NC
(This is a field-based role; personal vehicle required)
Shift & Schedule
- Shift: Day Shift
- Schedule: Monday – Friday
- Hours: 5x8-hour shifts
- Shift Time: 8:00 AM – 5:00 PM
- Weekends: As needed
- Holidays: Some holiday coverage expected
Key Responsibilities
- Manage and coordinate a full home health caseload
- Conduct comprehensive patient assessments in the home setting
- Develop, implement, and update individualized plans of care
- Provide skilled nursing care aligned with hospice and home health standards
- Educate patients and families on disease management and care plans
- Coordinate care with interdisciplinary team members
- Accurately document visits and outcomes using HomeCare HomeBase (HCHB)
- Ensure compliance with agency policies, state regulations, and best practices
Qualifications
Required
- Active RN license (temporary or pending accepted if license will be active by start date)
- Home Health travel experience required
- HomeCare HomeBase (HCHB) experience required
- Minimum 1 year of case management experience
- Valid driver’s license and reliable personal vehicle

- Charting System: HomeCare HomeBase (preferred)
- Travel Pairs: Considered on a case-by-case basis
- RTO Policy: Must be submitted with initial application; limited time off preferred
