JOB SUMMARY
The Medication Reconciliation Technician prepares detailed home medication lists utilizing a variety of tools. Duties include review of medical records for previous medication history, interview with patients and/or family members, communication with physician offices and/or pharmacies, and entry of medication information into the electronic medical record. Position may also assist pharmacy with day-to-day dispensing activities, record maintenance, patient assistance and medication replacement program when needed. Works under the supervision of a licensed pharmacist, practicing under the scope of State and Federal laws and statutes. Works as part of a larger care team based outside the pharmacy, primarily in the Emergency Department.
ESSENTIAL FUNCTIONS
- Medication Reconciliation including initial information uptake from patient, provider, LTC or pharmacy, review and check of the information for completeness and accuracy, follow up with necessary resources for any questions
- Medication replacement program assistance as determined.
- Patient assistance program liaison duties between pharmacy and case management
- Pyxis stocking and maintenance as needed.
- On a monthly basis, ensures checking of all appropriate locations for outdated medications.
- Answers telephone and refers all questions about medication orders, drug information, poison information or other clinical matters to the pharmacist on duty.
- Order management activities for all internal and external Departments serviced by the Pharmacy Department.
- Actively participates in the Quality program by assisting in identifying, documenting, monitoring, evaluating, and taking corrective action for quality improvement. Actively prepares for and participates in all regulatory inspections/surveys. Approaches work in a manner that makes quality improvement a part of every activity.
- Participates in cross-training experiences and continuing education as requested.
- Ensures that all expenditures for job related activities are within budgetary constraints.
- Responsible for the maintenance and proper operation of equipment within the department.
- Complies with the hospital’s Corporate Compliance Program including, but not limited to, the Code of Conduct, laws and regulations, and hospital and departmental policies and procedures.
- Must be free from governmental sanctions involving health care and/or financial practices.

JOB QUALIFICATIONS
- Education
- High school graduate or GED.
- Licensure
- Registered Licensed Pharmacy Technician with the Wyoming State Board of Pharmacy or eligible for Licensure as a Pharmacy Technician-in-Training by the Wyoming State Board of Pharmacy. Registered Licensed Pharmacy Technician required to be obtained within 2 years upon initial licensure of Pharmacy Technician-in-Training by the Wyoming State Board of Pharmacy.

- Experience
- Previous Retail Pharmacy Technician experience preferred.
Knowledge, Skills, and Abilities
- The ability to maintain continuing education requirements and all certifications and licenses as required (e.g. ACLS, PALS, CPI, CPR) to maintain professional license.
- 6 hours of ACPE continuing education annually and 20 hours biannually for CPhT requirements.
